Our customized plans allow our seniors to maintain as much independence as possible while receiving assistance with everyday activities.
We believe if we focus on activities that support a Cognitive, Physical, Nutritional and Social lifestyle it will promote lifelong benefits for our residents.
At Johnson Creek Village, we maximize our Resident’s quality of life by focusing on the person as a whole not just their brain impairments or diagnosis alone. In addition to their medical regimen, we use a variety of adjunctive therapies as an integral part of our holistic approach. We deliver care with Compassion, Dignity and Respect all while elevating our Residents’ quality of life. We adhere to our holistic philosophy through increased interaction and more personalized relationships between our Residents, their families and our staff. We also partner with community experts and specialists in various healthcare fields to provide exceptional and holistic care for our Residents.
On admission to Johnson Creek Village, we prepare a customized care plan that is meaningful to each Resident. We ask families to educate us about each Resident’s history to better understand the intricacies that make up their lives, their desire, goals and comforts.
Each staff member is provided with a copy of every Resident’s customized plan. They take great pride in getting to know each Resident in order to effectively assist them with everyday tasks like bathing, dressing, laundry, and housekeeping. Our staff is also trained to help manage medications and escort Residents to the dining room, activities, or events. Each resident’s living space features a 24-hour call system allowing Residents to receive help any time.
At Johnson Creek Village, we understand that most people hope to age safely and comfortably in their own home, including those managing dementia. Therefore, we take every measure possible to create an environment as close to home as possible. We have a policy of collecting memorable pieces of our Residents’ lives so they may bring them to their home away from home at Johnson Creek Village.
At Johnson Creek Village, Memory Care is our focus.
We create an environment customized for our Residents by discovering who they are, where they have been, and what they have done. By incorporating design, nutrition, and events with personal therapies, we help to understand and nurture our Residents during their challenges with dementia. Our goal is to foster an atmosphere of positive life experiences to take on the challenges of memory loss – with Compassion, Dignity and Respect.
Residents receive specialized memory care programming, along with all of the typical assisted living services, in a supportive environment. Our programming is designed to meet the needs of Residents as they progress through the stages of the disease.
Residents are encouraged to stay active and engaged by attending art therapy classes, movies and any of our numerous community events. Not everyone will be interested in the same activities, so we offer a variety of activities that embrace a wide range of interests.
At Johnson Creek Village, it’s important that your loved one continues to stay as active and participate socially as much as possible. Since we do focus on each Resident individually; we will incorporate the things that they love to do. What gives your loved one a sense of purpose?
Before moving into JCV, we will be asking these questions so we can better equip our staff with the knowledge to make each and every interaction customized to that Resident.

When a loved one is diagnosed with Alzheimer’s or other dementia, many people struggle to understand their condition. People tend to think that Alzheimer’s and dementia are interchangeable whereas they aren’t the same diagnosis. Dementia is not a specific disease; it’s a group of symptoms that affect mental and social abilities. It often is caused by conditions or changes in the brain. Alzheimer’s is a disease and the most common form of dementia. It’s a condition in which nerve cells in the brain die; making it hard for the brain to transmit signals properly.
